Richmond Elite Performance · 5335 Carey Avenue, Davenport, IA 52807 · Last updated: September 11, 2026

Draft — review before launch. This is a working template that covers the basics Meta requires for a lead-generation landing page. Have it reviewed by your attorney and confirm it matches how your CRM (GoHighLevel) and any other tools actually handle data.

1. Who we are

Richmond Elite Performance ("REP", "we", "us") operates this landing page to offer a free athlete performance assessment. Questions: [email protected] or (563) 723-8843.

2. What we collect

  • Information you give us through the form: athlete's first and last name, parent's first and last name, email, phone number, athlete age group, and the notes you provide about your athlete and their goals.
  • Marketing attribution automatically included with your submission: referring URL, page URL, and campaign parameters (e.g. utm_source, fbclid).
  • Usage data collected by cookies and similar technologies, including the Meta Pixel (see section 5).

3. How we use it

  • To contact you about scheduling your free assessment and about REP training programs.
  • To send follow-up messages by phone, SMS, and email. Message and data rates may apply. Reply STOP to opt out of texts, or use the unsubscribe link in any email.
  • To measure and improve our advertising.

4. Who we share it with

We do not sell your personal information. We share it only with service providers that help us operate — for example our CRM and messaging platform (GoHighLevel / LeadConnector) and our advertising platforms (Meta) — and only as needed to provide those services or as required by law.

5. Meta Pixel & advertising cookies

This page uses the Meta Pixel to understand ad performance and to build and measure audiences. Meta may receive information that you visited this page and that you submitted the form. You can manage ad personalization in your Meta account settings and control cookies through your browser.

6. Your choices

You may request access to, correction of, or deletion of your information by emailing [email protected]. You can opt out of marketing at any time as described in section 3.

7. Data retention & security

We keep lead information for as long as needed to follow up and for our ordinary business records, then delete or anonymize it. We use reasonable safeguards but no method of transmission or storage is 100% secure.

8. Children

This form is completed by a parent or guardian. We do not knowingly collect information directly from children through this page.

9. Changes

We may update this policy; the "last updated" date above will change accordingly.
